SEAL-A-SMILE DENTAL PROGRAM INFORMATION:
Marinette School District is working with Northern Health Centers Seal-A-Smile program along with the Marinette County Department of Health and Human Services to provide a preventive dental Sealant program for children in 2nd and 6th grade. Dental Sealants work best when placed on permanent molars, which begin to appear at approximately 6 years and 12 years of age.
This FREE program includes:
- Dental screening exam
- Fluoride varnish
- Sealants (if needed)
- Toothbrush and toothpaste
- Tooth brushing instructions and oral health education
- A letter sent home explaining what services were done and suggestions for further treatment
- Information on where to obtain additional dental care if needed

MEDICATIONS INFORMATION:
Please note there has been a change in Wisconsin's Medication law, see the Over-the-Counter section for more information**
Prescription Medication: If it is necessary for your child to receive prescription medication(s) during school hours, a specific school district form must be completed. For prescription medication(s), a parent/guardian signature and the signature of the prescribing doctor is required. All prescription medications must be brought to school in the original pharmacy-labeled container.
Over-the-Counter Medication: Due to a recent law change we will no longer stock basic over-the-counter medications (such as: acetaminophen (tylenol), ibuprofen (advil), caladryl, benadryl, neosporin or cough/throat drops) as we have done in the past. If you wish for your child to have access to these types of medications during the school year you must provide the medication(s). All over-the-counter medications must be in the original packaging. Please ask for the form that you need from the health room assistant or secretary at the school where your child attends or will attend.

STUDENT IMMUNIZATIONS:

The School Nurse facilitates the following:
- positive student responses to normal development;
- promotes health and safety;
- intervenes with actual and potential health problems;
- provides case management services;
- actively collaborates with others to build student and family capacity for adaptation, self-management, self-advocacy, and learning.
The School Nurse's role as part of the educational team is...
- Develop Individualized Health Plans and Action Plans for students.
- Take part in the process of developing Individualized Education Plan for students.
- Provide in-service training for teachers and staff regarding the individual health needs of students.
- Assist parents/guardians/teachers to identify and utilize community resources.
- Coordinate the services through training, delegation and supervision of staff.

ATHLETIC INJURIES:
Medical providers need a form filled out to release personal information to coaches and administration of the Marinette School District. Parents can read/print a copy directly from Bellin's website at: http://www.bellin.org/hipaaforms
Parents should read all of the information, print and sign the form, and then turn it in at either the Marinette High School or Marinette Middle School (depending on where their child attends).
